About Horizon Connections

At Horizon Connections Community Care Services, LLC, we believe adulthood should offer opportunities to make friends, discover interests, build confidence, and participate in community life.

Our adult day program in Apple Valley is being developed to support adults with developmental disabilities through meaningful activities and practical learning. We recognize each person’s strengths, preferences, and goals—and believe they should have a voice in shaping their day.

Your interests. Your choices. Your goals.

Progress looks different for everyone. It may mean preparing a snack, communicating a preference, learning a new task, or making a friend. We value each accomplishment.

Our approach centers on dignity, respect, and opportunities to make choices. We aim to work alongside participants, families, authorized representatives, and support teams to create experiences that are meaningful to each person.

Meaningful Days and Skills for Everyday Life

Our planned program brings together learning, creativity, movement, and connection. Opportunities will include:

  • Cooking and meal preparation: Practice preparing meals, following recipes, kitchen safety, and making nutritious choices.

  • Gardening: Plant, grow, and care for a garden while enjoying hands-on activities and time outdoors.

  • Daily living skills: Build confidence with personal routines, organization, money skills, and everyday problem-solving.

  • Vocational skills: Explore interests and practice communication, teamwork, task completion, and habits that support future work opportunities.

  • Art and creative expression: Discover painting, crafts, music, and other ways to express ideas and celebrate individual talents.

  • Friendship and community participation: Practice social skills, develop connections, and explore opportunities to participate in the community.

  • Wellness and recreation: Enjoy movement, games, relaxation, and activities adapted to different interests and abilities.

We also plan to offer access to speech therapy, occupational therapy, and counseling, based on individual needs, qualified provider availability, and applicable service authorizations. These supports can help participants work toward communication, everyday functioning, and emotional wellness goals.
